Consultation & Consent Form
Consultation & Consent Form
• Every treatment starts with a consultation where you will be asked to complete a consent form for the treatment. If one is not completed the treatment will not be carried out.
• This consent form outlines the aftercare you will need to follow for the best possible results. We will not be held responsible for poor retention especially where aftercare is not adhered to.
• The consent form asks for permission to use photos and videos taken during the treatment on social media and for marketing purposes, if permission is not given this will be fully respected. Where photos and videos are used no royalties will be paid.
• Your consent form explains this is a two part process, the cost includes your initial treatment, a retouch 6-12 weeks later and aftercare products. If the retouch is not taken in this time an extra cost will be incurred, therefore we advise you to book this at the end of your initial treatment.
• You need both treatments to complete the process, You have the right to discontinue treatment at any time however no part refunds will be given if you do not have your retouch appointment.
• Some clients may require a third treatment, this is usually clients with oiler skin or that are on medication. A third retouch will be charged at £75
• The possible risk involved will be discussed, these may include but are not limited to bruising, swelling, allergic reactions, bleeding, infection and scarring.
• A medical history form will be filled out disclosing any medication or medical issues. Your safety is my utmost priority. Some medications or issues may prevent the treatment being carried out or may require GP consent. Understand that some medications can cause poor pigment retention
• You will sign to confirm the results from the patch test before anything is carried out.
• All pre-procedure advice must be adhered to.
• Mapping consent will be signed once you are happy with the design. You will accept the result of Your Treatment and/or Follow Up Treatment without any legal recourse to Us on the understanding that You will pre-approve both the shape and colouring of the Permanent Make Up at the beginning of Your Treatment.
